Uncommon procedure worth sharing for awareness!

Had an emergency case earlier,

Bleeding all over patients mouth and mobile upper canine and premolar.

Constant spitting and gargling prior consult. No pressure application.

As Narrated by the wife. " uyog na kaayo ang ngipon gi-try niyag ibot doc pag gabie nya wala makuha mao pag kadlawon nagsige nag dugo hantod Rong buntag. "

Blood is seen from his face to his neck ,the mask is covered from fresh blood, the feet had blood splatters and on his hands and shirt.

He is covering his mouth, i thought it's trauma then I decided to check , before referring to a hospital nearby,

That's my first plan, if it's not my capability to do the case then I'll refer them after bleeding control.

The wife is very anxious seeing her husband spitting non-stop mostly fresh blood. I was hesitant to accept the case but it needs prompt treatment.

They visited me hoping I could help because I'm the nearest clinic.

Patient is in pain, stated it bleed since 1 o'clock the morning.

I composed myself nobody cant help them as of the moment, though my legs are trembling seeing the amount of blood in the cuspidor and his mouth which I don't normally see in our set up.

The patient needed to visit the hospital in my first assessment but the longer they wait , the risks is very high for him to loose more blood.

It's my call to decide!

Then I gave the patient 2 capsules of tranexamic medications, irrigated with lots of NSS , remove all blood clots, wiped with betadine

Then I administered local anesthetics, gently pulled the mobile teeth,

Then I applied pressure, placed anti-bleeding solution on the site.

Then I sutured it. I let the patient bit on some gauze.

I prescribed some antibiotics and added some emergency instructions and possible visit for blood check he might have underlying problems.

I did what I think possible on my head, I just followed what's appropriate and fast.

I called around 4pm, then the wife said his sleeping and no more bleeding and pain.

I thank God for the gift of knowledge and a brave heart to save my patient.
